Short answer

As shown on Oct 21, 2026, Lilly's Foundayo savings card lets commercially insured patients whose plan covers Foundayo pay as little as $25 per fill, saving up to $100 per 1-month fill and $1,000 a year. Example: a $125 copay minus $100 leaves $25. Without insurance, LillyDirect self-pay starts at $149 a month.

Foundayo savings and price at a glance

Foundayo (orforglipron) is Lilly’s once-daily GLP-1 tablet for chronic weight management, approved by the FDA on Apr 1, 2026 (FDA, Apr 1, 2026). What you pay depends mostly on your coverage. Every price below is as shown on Lilly’s Foundayo pages on Oct 21, 2026. Programs change; confirm on Lilly’s site before you fill.

The $25 card applies when your plan covers Foundayo. Lilly’s terms also cover commercial plans that do not cover Foundayo: as low as $149 (0.8 or 2.5 mg), $199 (5.5 mg) or $299 (9, 14.5 or 17.2 mg) per 1-month fill, with monthly savings capped at $520, $470 or $370 by strength, up to 10 fills a year, expiring 12/31/2026 (Eli Lilly, Oct 21, 2026).

How the commercial savings card works

The card pays part of your out-of-pocket cost at the pharmacy, up to a cap, and you pay the rest. As shown on Oct 21, 2026, the card brings a covered fill down to as little as $25 (Eli Lilly, Oct 21, 2026).

Foundayo commercial savings card limits, as shown on Oct 21, 2026 (Eli Lilly, Oct 21, 2026)
Fill lengthMaximum savings per fillLowest price listed
1-month fill$100As little as $25
2-month fill$200As little as $25
3-month fill$300As little as $25
Per calendar year$1,000Up to 10 fills

Two worked examples, which are our arithmetic and not Lilly figures:

  • 1-month fill. If your plan’s copay is $125, the card covers $100 and you pay $25. If your copay is $175, the card still covers only $100, and you pay $75.
  • 3-month fill. If your copay for a 3-month supply is $325, the card covers $300 and you pay $25.

The annual cap of $1,000 equals 10 one-month fills at the $100 maximum (10 × $100), which matches the 10-fill limit. Longer fills reach the annual cap sooner: four 3-month fills at the $300 maximum would add up to $1,200, which is more than the $1,000 a year the card allows. If you use 3-month fills, expect the last fill of the year to receive less help.

Government-program enrollees, including Medicare, Medicaid, TRICARE and VA, are excluded from this card (Eli Lilly, Oct 21, 2026).

LillyDirect self-pay prices by strength

LillyDirect is Lilly’s own online pharmacy service. Self-pay means you pay cash instead of billing insurance. As shown on Oct 21, 2026, LillyDirect listed Foundayo prices by tablet strength, and Lilly’s self-pay terms list a regular price for each strength (Eli Lilly, Oct 21, 2026) (Eli Lilly, Oct 21, 2026).

Foundayo LillyDirect self-pay price per month, current and regular, as shown on Oct 21, 2026 (Eli Lilly, Oct 21, 2026) (Eli Lilly, Oct 21, 2026)
StrengthPrice shown Oct 21, 2026Regular priceCondition
0.8 mg$149$149None listed
2.5 mg$149$199Temporarily reduced through Dec 31, 2026
5.5 mg$199$299Temporarily reduced through Dec 31, 2026
9 mg$299$299None listed
14.5 mg$299$349When you refill within 45 days
17.2 mg$299$349When you refill within 45 days

Three points stand out. The two lowest strengths share the same $149 price through the end of 2026. The temporary reductions save $50 a month at 2.5 mg and $100 a month at 5.5 mg compared with the regular prices. And at the top of the range, 9 mg, 14.5 mg and 17.2 mg all show $299, as long as the 45-day refill condition is met for the two highest strengths.

Lilly’s self-pay terms state that the self-pay card expires on 12/31/2026 (Eli Lilly, Oct 21, 2026). We found no published self-pay price for fills after that date.

What the 45-day refill condition means

For 14.5 mg and 17.2 mg, the $299 price applies “when you refill within 45 days” (Eli Lilly, Oct 21, 2026), counted from the delivery or received date of your previous Foundayo fill (Eli Lilly, Oct 21, 2026). Lilly’s self-pay terms list $349 as the regular price for both strengths, a $50 difference per fill (Eli Lilly, Oct 21, 2026).

Each bottle holds 30 tablets, and the label says to take one tablet once daily (Eli Lilly, Jul 29, 2026). That leaves about 15 days between the end of a bottle and the end of the 45-day window. A delayed prescription renewal, a pause in treatment or a shipping delay can push a refill past that window. The condition is not listed for 0.8 mg, 2.5 mg, 5.5 mg or 9 mg.

List price: the $649 WAC and what it means

Lilly’s Connecticut price disclosure sheet lists a wholesale acquisition cost (WAC) of $649.00 per 30-tablet bottle for every Foundayo strength, per AnalySource accessed 04/02/2026 (Eli Lilly, Apr 2, 2026).

WAC is the manufacturer’s published list price to wholesalers, before any rebates or discounts. It is not necessarily what you pay: your price depends on your coverage or on the program you use, such as the savings card or LillyDirect self-pay. If your plan charges coinsurance as a percentage of the drug price, ask the plan which price it uses.

For scale, the regular self-pay price for the top strengths ($349) is $300 below the $649 WAC, and the lowest self-pay price ($149) is $500 below it. Twelve bottles at WAC would total $7,788, which is our arithmetic, not a Lilly figure.

Medicare and the GLP-1 Bridge

The Foundayo savings card excludes Medicare (Eli Lilly, Oct 21, 2026). Lilly’s Jun 25, 2026 release describes a separate route for Part D patients (Eli Lilly, Jun 25, 2026):

  • Price. $50 a month, with prior authorization, for eligible Part D patients using Foundayo for weight management.
  • Dates. The Bridge runs from Jul 1, 2026 through Dec 31, 2027.
  • Clinical criteria. BMI of 35 or more, or BMI of 27 or more with certain conditions.
  • Not eligible under this pathway. Patients already getting a GLP-1 through their Part D plan, and patients with type 2 diabetes, moderate-to-severe obstructive sleep apnea or fatty liver disease (Lilly notes a Part D plan may already cover those conditions).

At $50 a month, 12 months would come to $600, which is our arithmetic. Ask your Part D plan or your prescriber whether you meet the criteria and how prior authorization works for your plan.

Example: 12 months of self-pay on the label schedule

This is our arithmetic, not a Lilly figure, and it is not a dosing recommendation. It shows what the published prices add up to if someone followed every step of the label schedule at the shortest interval the label allows.

The label says the starting dosage is 0.8 mg once daily, increased to 2.5 mg after at least 30 days, then to 5.5 mg after at least 30 days on 2.5 mg. After at least 30 days at the current dose, the dose may go up to 9, 14.5 or 17.2 mg, depending on treatment response and tolerability. The maximum is 17.2 mg once daily (Eli Lilly, Jul 29, 2026). Many people will not move up every 30 days, or at all, so read this as an upper-path example.

Assumptions:

  • One 30-tablet bottle equals one month, and each step is held exactly 30 days, so 12 bottles cover 360 days.
  • The “shown Oct 21, 2026” column applies those prices to all 12 fills, and every refill happens within 45 days.
  • The self-pay card terms cap use at 10 prescription fills per calendar year (Eli Lilly, Oct 21, 2026). Twelve fills inside one calendar year would exceed that cap, and we found no published price for fills beyond it, so treat the 12-month totals as arithmetic on the listed prices, not a quote.
  • The temporary 2.5 mg and 5.5 mg prices end Dec 31, 2026, and the self-pay card expires the same day. The “regular” column shows the same path at Lilly’s regular prices as a reference, not a forecast. Lilly has not published 2027 prices.
Our arithmetic, 12 months of LillyDirect self-pay on the label ladder, prices shown Oct 21, 2026 (excludes tax and shipping) (Eli Lilly, Oct 21, 2026) (Eli Lilly, Oct 21, 2026)

6 to 1217.2 mg (7 fills)$2,093$2,443
12-month total12 fills$3,188$3,738

At the prices shown on Oct 21, 2026, the 12-month path totals $3,188, an average of about $266 a month. At regular prices it totals $3,738, about $312 a month. The $550 gap comes from the temporary 2.5 mg and 5.5 mg reductions ($150) and the 45-day refill price on 14.5 mg and 17.2 mg ($400 across 8 fills). Staying at a lower strength changes the total; at 9 mg for months 4 to 12, the total would be the same $3,188 at the prices shown, because 9 mg, 14.5 mg and 17.2 mg all list $299.

For the full label schedule, missed-dose rules and interactions that can cap the dose, see the Foundayo dosing schedule. Talk to your prescriber about which strength and pace fit you.

Fine print before a fill

  • Terms can change. These are promotional programs. Several terms end on 12/31/2026, so re-verify prices and eligibility on foundayo.lilly.com and LillyDirect before each fill (Eli Lilly, Oct 21, 2026).
  • The card needs coverage. The commercial card is for patients whose plan covers Foundayo. Your plan’s formulary, prior authorization rules and deductible decide your copay before the card applies.
  • Fill limits. The card allows up to 10 fills and $1,000 in savings per calendar year (Eli Lilly, Oct 21, 2026).
  • Government programs are excluded from the savings card. Medicare Part D patients can look at the Bridge instead (Eli Lilly, Jun 25, 2026).
  • One product. Foundayo is the FDA-approved orforglipron product (NDA 220934) (FDA, Apr 1, 2026); compounded or “research” orforglipron is not FDA approved and is not interchangeable with Foundayo.
  • Same price for every strength at list. The $649.00 WAC does not change with strength, but self-pay prices do (Eli Lilly, Apr 2, 2026).

Comparing other GLP-1 programs

If you are weighing Foundayo against Novo Nordisk’s oral semaglutide, our Foundayo vs Wegovy pill comparison sets the two side by side. For Wegovy’s manufacturer programs, see the Wegovy savings card guide, and for Lilly’s injectable, the Zepbound savings card guide. Our calculators cover the injectable brands.

This page reports program terms; it does not recommend a route. Talk to your prescriber or pharmacist about which option fits your coverage and prescription.

Frequently asked questions

As shown on LillyDirect on Oct 21, 2026, self-pay prices are $149 a month for 0.8 mg and 2.5 mg, $199 for 5.5 mg, $299 for 9 mg, and $299 for 14.5 mg and 17.2 mg when you refill within 45 days. The 2.5 mg and 5.5 mg prices are temporarily reduced through Dec 31, 2026. Lilly lists regular prices of $199 (2.5 mg), $299 (5.5 mg and 9 mg) and $349 (14.5 mg and 17.2 mg). Confirm current prices on Lilly's site.

Is there a Foundayo savings card?

Yes. Lilly's Foundayo savings card, as shown on Oct 21, 2026, is for patients whose commercial plan covers Foundayo. You may pay as little as $25 for a 1-, 2- or 3-month fill. Savings are capped at $100, $200 or $300 per fill and $1,000 per calendar year, with up to 10 fills a year. If your commercial plan does not cover Foundayo, the terms list prices as low as $149 to $299 per 1-month fill by strength. Medicare, Medicaid, TRICARE, VA and other government-program enrollees are excluded.

Does Medicare cover Foundayo?

The savings card excludes Medicare. Separately, Lilly's Jun 25, 2026 release describes the Medicare GLP-1 Bridge: eligible Part D patients using Foundayo for weight management may pay $50 a month with prior authorization from Jul 1, 2026 through Dec 31, 2027. The clinical criteria are a BMI of 35 or more, or 27 or more with certain conditions. Patients already getting a GLP-1 through their Part D plan, and those with type 2 diabetes, moderate-to-severe obstructive sleep apnea or fatty liver disease, are not eligible under this pathway (a Part D plan may already cover those conditions).

How much is Foundayo per month?

It depends on coverage and strength. With a commercial plan that covers Foundayo, the savings card price is as little as $25 per fill. Without insurance, LillyDirect self-pay ranged from $149 to $299 a month as shown on Oct 21, 2026. The Medicare GLP-1 Bridge is $50 a month for eligible Part D patients. Lilly's published list price (WAC) is $649.00 per 30-tablet bottle for every strength.

When does the Foundayo savings card expire?

Lilly's Foundayo coverage and savings terms, viewed Oct 21, 2026, state that both the commercial savings card and the self-pay card expire on 12/31/2026. The temporarily reduced 2.5 mg and 5.5 mg self-pay prices also run only through Dec 31, 2026. Program terms can change, so check foundayo.lilly.com before each fill.
