Short answer

As shown on Oct 19, 2026, Lilly's Zepbound savings card cuts a covered commercial fill of the single-dose pen to as little as $25, saving up to $100 per 1-month fill and $1,300 a year. Example: a $125 copay minus $100 leaves $25. Without coverage, the card price is $499 a month. Government-insured patients are excluded.

The Zepbound savings card at a glance

Lilly runs more than one Zepbound savings card, and which one applies depends on your insurance and on which presentation is prescribed. Every price below is as shown on Lilly’s pages on Oct 19, 2026, and rechecked on Oct 20, 2026. Programs change; confirm on Lilly’s site before you fill.

The page code on the savings page reads CMAT-26402 09/2026, which dates the version reviewed for this page (Eli Lilly, Sep 1, 2026). The covered-plan card applies to the single-dose pen. The KwikPen has its own card. The not-covered price of $499 is for a 1-month fill only.

One pattern is worth noticing: the covered-plan annual cap of $1,300 equals 13 one-month fills at the $100 monthly maximum (13 × $100). The yearly cap is not a separate surprise. It is the monthly cap applied to the full 13 fills the card allows.

Which route fits which situation

This is a map of what Lilly lists for each situation, not a recommendation. Your plan’s formulary, prior authorization rules and deductible decide what you actually pay at the counter. Your prescriber or pharmacist can confirm which presentation is on your prescription.

Medicare GLP-1 Bridge and CVS Caremark

Two coverage routes sit outside the savings cards. Both are as shown on Lilly’s savings page on Oct 20, 2026 (Eli Lilly, Sep 1, 2026).

  • Medicare GLP-1 Bridge. Lilly says eligible Medicare Part D patients with a Zepbound prescription for weight management may get Zepbound for no more than $50 a month. Eligibility is based on the Medicare GLP-1 Bridge clinical criteria, and Lilly states it is not a guarantee of coverage. LillyDirect shows the Bridge next to the KwikPen (Eli Lilly, Oct 19, 2026). The savings cards themselves still exclude Medicare.
  • CVS Caremark. Lilly says coverage for Zepbound has been restored on select CVS Caremark formularies, starting at $25 a month. Coverage is not guaranteed, and the plan administrator decides actual benefits. Governmental beneficiaries are excluded from the $25 price.

Ask Lilly, your Part D plan or your pharmacy benefit manager whether either route applies to you.

LillyDirect self-pay: Zepbound KwikPen and single-dose vial prices

LillyDirect is Lilly’s own online pharmacy service. Self-pay means you pay cash rather than billing insurance. On Oct 19, 2026, LillyDirect showed identical self-pay prices for the multi-dose KwikPen and the single-dose vials (Eli Lilly, Oct 19, 2026).

LillyDirect Zepbound self-pay price per 28 days, as shown on Oct 19, 2026 (Eli Lilly, Oct 19, 2026)
DoseKwikPen (1 pen, 4 doses)Single-dose vials (4 vials)If not refilled within 45 days
2.5 mg$299$299No change listed
5 mg$399$399No change listed
7.5 mg$449$449$499
10 mg$449$449$699
12.5 mg$449$449$699
15 mg$449$449$699

What the page defines:

The prices step with the dose because the label escalates in 2.5 mg steps. See the Zepbound dosing schedule for the label ladder and the tirzepatide titration tool to lay it out by date.

What the 45-day refill rule means

The $449 price for 7.5, 10, 12.5 and 15 mg holds only if the prescription is refilled within 45 days. If more time passes, LillyDirect lists regular prices of $499 for 7.5 mg and $699 for 10, 12.5 and 15 mg (Eli Lilly, Oct 19, 2026). That is a $50 difference at 7.5 mg and a $250 difference at 10 to 15 mg per 28-day fill.

As listed on Oct 19, 2026, the 2.5 mg and 5 mg prices carry no 45-day condition. Each fill covers 28 days, so the 45-day window leaves 17 days of slack after a fill runs out. A delayed refill, a gap in treatment or a prescription renewal that takes longer than expected can push a fill past the window.

How Zepbound self-pay prices changed: a dated history

Lilly has changed Zepbound self-pay pricing several times. These are the dated steps we could confirm from Lilly’s own releases and pages.

Zepbound self-pay pricing milestones from Lilly sources (Eli Lilly, Dec 1, 2025) (Eli Lilly, Feb 23, 2026) (Eli Lilly, Mar 16, 2026)
DateWhat changedSource
Dec 1, 2025Single-dose vial self-pay cut: 2.5 mg $349 to $299; 5 mg $499 to $399; 7.5–15 mg $499 to $449Lilly release
Feb 23, 2026FDA label expansion adds the 4-dose KwikPen; KwikPen self-pay launches at LillyDirect from $299Lilly release
Mar 16, 2026KwikPen self-pay pricing extended to major retail pharmacies through a KwikPen Self-Pay Savings CardLilly statement
Oct 19, 2026KwikPen and vials both listed at $299 / $399 / $449, with $499 / $699 regular pricesLillyDirect page

The Dec 1, 2025 release also listed higher regular vial prices for people outside the refill program: $599 (7.5 mg), $699 (10 mg), $849 (12.5 mg) and $1,049 (15 mg) (Eli Lilly, Dec 1, 2025). The LillyDirect page we read on Oct 19, 2026 shows $499 and $699 instead. We report the current page and date it.

Lilly’s March 16, 2026 statement is specific: the retail pharmacy card “applies only to the Zepbound KwikPen” and not to single-dose vials or the single-dose pen (Eli Lilly, Mar 16, 2026).

GoodRx coupons for Zepbound

Search results often point to GoodRx coupons for Zepbound. On Oct 19, 2026, the GoodRx Zepbound page (prices “last updated on 10/19/26”) showed a Lilly-sponsored “manufacturer offer” of $299 a month for the 2.5 mg KwikPen, self-pay only, with pharmacies listed at $299.00 (GoodRx, Oct 19, 2026). That is the same $299 KwikPen price Lilly lists. It is not a separate GoodRx-negotiated discount on Zepbound. We did not find a lower GoodRx price to report.

Annual totals at the listed prices

Monthly prices can hide the yearly number. This table multiplies the prices Lilly listed on Oct 19, 2026 by 13 fills. Thirteen is the fill limit on the single-dose pen cards. For LillyDirect it is our arithmetic for one year of 28-day months, not a Lilly figure.

Our arithmetic, 13 fills at prices shown Oct 19, 2026 (single dose held all year; excludes needles, syringes and tax) (Eli Lilly, Oct 19, 2026) (Eli Lilly, Sep 1, 2026)
Route and dosePer fill13 fills
Self-pay 2.5 mg$299$3,887
Self-pay 5 mg$399$5,187
Self-pay 7.5–15 mg, refilled within 45 days$449$5,837
Self-pay 10–15 mg, regular price$699$9,087
Savings card, plan does not cover the pen$499$6,487

Real-world totals will differ. Under the label, 2.5 mg is used for the first 4 weeks only, and the dose often changes during the year (DailyMed, Aug 28, 2026). The tirzepatide cost calculator can lay out the label ladder week by week and apply a price you enter.

Fine print worth reading before a fill

  • Terms can change at any time. Lilly says its program terms may be terminated, rescinded, revoked or amended without notice (Eli Lilly, Sep 1, 2026).
  • Eligibility is required. The savings page notes “Eligibility required, terms and conditions apply,” with monthly and annual limits (Eli Lilly, Sep 1, 2026).
  • Cards are presentation-specific. The covered-plan card is for the single-dose pen. The KwikPen has its own card. The retail KwikPen card does not apply to vials or the single-dose pen (Eli Lilly, Mar 16, 2026).
  • KwikPen card limits. For each KwikPen dose, a month is 28 days and up to 1 KwikPen. The card covers up to 11 fills a calendar year. Post-purchase reimbursement claims must be submitted or postmarked by March 31, 2027 (Eli Lilly, Sep 1, 2026).
  • The KwikPen holds 4 doses. The label lists the KwikPen and multi-dose vial as 4-dose presentations, so one KwikPen covers 4 weekly doses (DailyMed, Aug 28, 2026).
  • Compounded tirzepatide is a different product. The FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed (FDA, Oct 1, 2026). Lilly’s programs on this page are for Lilly’s Zepbound presentations, and compounded products are not interchangeable with Zepbound.

Storage rules differ by presentation as well, which matters if you switch from vials to a KwikPen. See the tirzepatide storage tool and the tirzepatide hub for label facts on each form. If you use Mounjaro instead, its card has different caps; see the Mounjaro savings card guide.

Talk to your prescriber or pharmacist about which presentation and dose fit your prescription. This page reports program terms; it does not recommend a route.

Frequently asked questions

How much is Zepbound with the savings card?

As shown on Lilly's Zepbound savings page on Oct 19, 2026: if your commercial plan covers the single-dose pen, you may pay as little as $25 for a 1-, 2- or 3-month fill, with savings capped at $100, $200 or $300 per fill and $1,300 per calendar year. If your commercial plan does not cover it, the card price is as low as $499 for a 1-month fill. Programs change; confirm on Lilly's site.

Is there a Zepbound discount card?

Lilly's own Zepbound savings card is the manufacturer program. As shown on Oct 19, 2026, there is a card for the single-dose pen (covered and not-covered commercial plans) and a separate KwikPen card listed at $299, $399 and $449 per 1-month fill depending on dose. Both exclude government beneficiaries and expire Dec 31, 2026. Confirm current terms on zepbound.lilly.com.

Can I use GoodRx for Zepbound?

On Oct 19, 2026, GoodRx's Zepbound page showed a Lilly-sponsored manufacturer offer of $299 a month for the 2.5 mg KwikPen, self-pay only, with pharmacies listed at $299.00. That is Lilly's KwikPen self-pay price shown on GoodRx, not a separate GoodRx-negotiated discount. Prices on third-party sites change often.

How much does Zepbound cost on LillyDirect in 2026?

As shown on LillyDirect on Oct 19, 2026, self-pay prices for the KwikPen and the single-dose vials are the same: $299 for 2.5 mg and $399 for 5 mg per 28 days, and $449 for 7.5, 10, 12.5 or 15 mg if the prescription is refilled within 45 days. Otherwise the regular price is $499 for 7.5 mg and $699 for 10 to 15 mg.

Can I use the Zepbound savings card with Medicare?

No. The Zepbound savings page says governmental beneficiaries are excluded, which covers programs such as Medicare and Medicaid. Separately, as shown on Oct 20, 2026, Lilly lists coverage through the Medicare GLP-1 Bridge for no more than $50 a month, for eligible Part D patients prescribed Zepbound for weight management. Eligibility follows the Bridge clinical criteria and coverage is not guaranteed.

When does the Zepbound savings card expire?

The Zepbound savings page, viewed Oct 19, 2026, states that the card expires and savings end on 12/31/2026. Lilly says terms may be changed or ended at any time without notice, so confirm current terms on zepbound.lilly.com before a fill.
