Short answer
As shown on Oct 19, 2026, the Mounjaro savings card lets people with commercial coverage for the single-dose pen pay as little as $25, saving up to $150 per 1-month fill and $1,950 a year. Example: a $175 copay minus $150 leaves $25. Without coverage, the price is as low as $499 a month.

The Mounjaro savings card at a glance
Lilly’s Mounjaro savings card has two tiers. Which one applies depends on whether your commercial plan covers Mounjaro. Every figure below is as shown on Lilly’s Mounjaro savings page on Oct 19, 2026. Programs change; confirm on Lilly’s site before you fill.
| Your coverage | You pay | Maximum savings | Fills per year |
|---|---|---|---|
| Commercial plan covers the single-dose pen | As little as $25 for a 1-, 2- or 3-month fill | $150 (1-month), $300 (2-month), $450 (3-month) per fill; $1,950 per calendar year | 13 |
| Commercial plan does not cover Mounjaro | As low as $499 for a 1-month fill | $647 per month; $8,411 per calendar year | Up to 13 |
| Medicare, Medicaid or other government insurance | Not eligible | Not applicable | Not applicable |
| All tiers | Single-dose pen NDCs only | Card expires 12/31/2026 | Not applicable |
The card covers the single-dose pen. The current US label lists four presentations: single-dose pen, single-dose vial, multi-dose vial and single-patient-use KwikPen (DailyMed, Aug 27, 2026). The savings terms we reviewed name only the single-dose pen (Eli Lilly, Oct 19, 2026).
How the Mounjaro caps work
Both annual caps line up exactly with 13 fills at the monthly maximum. For a covered plan, 13 × $150 = $1,950. For a plan that does not cover Mounjaro, 13 × $647 = $8,411. So the yearly limit is the monthly cap applied to every fill the card allows.
The worked examples below are our arithmetic on the published caps. They are not quotes from Lilly, and your plan’s actual copay sets the result.
| Example | Pharmacy charge to you before card | Card pays (up to cap) | You pay |
|---|---|---|---|
| Covered plan, 1-month fill | $175 copay | $150 | $25 |
| Covered plan, 3-month fill | $475 copay | $450 | $25 |
| Covered plan, 1-month fill, high copay | $300 copay | $150 | $150 |
| Not-covered plan, 1-month fill | $1,146 cash price | $647 | $499 |
The last row shows the edge of the not-covered tier: $499 plus the $647 cap is $1,146. A 1-month cash price above that would leave you paying more than $499. The third row shows the same edge for a covered plan: a copay above $175 for a 1-month fill leaves more than $25 to pay.
The tirzepatide cost calculator totals a schedule over any number of weeks at a price you enter.
Which route fits which situation
This table maps what Lilly lists to common coverage situations. It is not a recommendation. Your formulary, prior authorization and deductible determine what you pay, and your pharmacist can run the card against your claim.
| Situation | What Lilly lists | Price shown Oct 19, 2026 | Key condition |
|---|---|---|---|
| Commercial insurance covers Mounjaro | Savings card | As little as $25 per 1-, 2- or 3-month fill | Single-dose pen; $1,950 annual cap |
| Commercial insurance does not cover Mounjaro | Savings card, not-covered tier | As low as $499 per 1-month fill | $647 monthly and $8,411 annual caps |
| Paying out of pocket (self-pay) | LillyDirect, single-dose pen | $499 a month, all strengths | Prices are Lilly’s self-pay list |
| Medicare, Medicaid or other government plan | Savings card excluded | Not applicable | Plan rules apply |
LillyDirect self-pay price for Mounjaro
On Oct 19, 2026, LillyDirect listed the Mounjaro single-dose pen at $499 a month for every strength (Eli Lilly, Oct 19, 2026). Unlike Zepbound self-pay pricing, the Mounjaro price does not step with the dose.
| Strength | Self-pay price per month | 13 months at that price (our arithmetic) |
|---|---|---|
| 2.5 mg | $499 | $6,487 |
| 5 mg | $499 | $6,487 |
| 7.5 mg | $499 | $6,487 |
| 10 mg | $499 | $6,487 |
| 12.5 mg | $499 | $6,487 |
| 15 mg | $499 | $6,487 |
A flat price means a dose change does not change the self-pay cost.
Yearly cost at the listed prices
Monthly prices can hide the yearly total. This table multiplies the lowest listed price on each route by 13 fills, the card’s yearly fill limit. It is our arithmetic, not a Lilly figure, and it assumes every fill qualifies at the lowest price.
| Route | Lowest price per 1-month fill | 13 fills |
|---|---|---|
| Savings card, covered plan | $25 | $325 |
| Savings card, plan does not cover Mounjaro | $499 | $6,487 |
| LillyDirect self-pay, any strength | $499 | $6,487 |
The not-covered card price and the LillyDirect self-pay price are the same $499 on the pages we reviewed. What differs is the mechanism: one is a card applied at a pharmacy, the other is a cash price through Lilly’s own service. The label escalates in 2.5 mg steps after at least 4 weeks on the current dose when more glycemic control is needed, up to 15 mg in adults (DailyMed, Aug 27, 2026). See the Mounjaro dosing schedule and the tirzepatide titration tool for the label ladder.

| Covered plan, annual cap | $1,950 | $1,300 |

What Mounjaro is approved for
The current label lists two indications (DailyMed, Aug 27, 2026):
- As an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients 10 years of age and older with type 2 diabetes.
- To reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes who are at high cardiovascular risk.
Lilly’s May 13, 2022 approval release stated that Mounjaro is not indicated for weight loss (Eli Lilly, May 13, 2022). Zepbound is the tirzepatide brand labeled for chronic weight management. The tirzepatide hub covers both brands.
The label lists six strengths for the single-dose pen: 2.5, 5, 7.5, 10, 12.5 and 15 mg, each in 0.5 mL (DailyMed, Aug 27, 2026).
Fine print worth reading before a fill
- Terms can change. Lilly states its program terms may be terminated, rescinded, revoked or amended at any time without notice (Eli Lilly, Oct 19, 2026).
- Government beneficiaries are excluded from both tiers (Eli Lilly, Oct 19, 2026).
- Single-dose pen only. The card applies to Mounjaro single-dose pen NDCs (Eli Lilly, Oct 19, 2026).
- GoodRx. We could not verify GoodRx’s Mounjaro page on Oct 19, 2026, so we report no GoodRx price for Mounjaro.
- Compounded tirzepatide is a different product. The FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed (FDA, Oct 1, 2026). Compounded products are not FDA approved and are not interchangeable with Mounjaro.
Storage rules differ by presentation; the tirzepatide storage tool lists the label limits. Talk to your prescriber or pharmacist about coverage, dose and presentation. This page reports program terms and does not recommend a route.
