Short answer

Week of step k = 1 + weeks per step × (k − 1). From a Jan 5, 2026 start, 5 mg begins week 5 (Feb 2) and, stepping every 4 weeks, 15 mg begins week 21 (May 25). It shows the FDA label ladder, not a recommendation; your prescriber sets your steps. Dates as of Oct 19, 2026.

FDA labels set a minimum of 4 weeks per step; a prescriber may hold a step longer. The table assumes exactly 4 weeks so the dates are a best case, not a plan.

Frequently asked questions

What is the tirzepatide dose schedule on the label?

The Zepbound and Mounjaro labels start at 2.5 mg once weekly for 4 weeks, then 5 mg, with increases in 2.5 mg steps after at least 4 weeks on the current dose, to a maximum of 15 mg. Zepbound lists 5, 10 or 15 mg maintenance for weight reduction.

Can the weeks per step be changed?

Yes, the calculator accepts any number of weeks per step. The published label uses at least 4 weeks per step; labels allow a prescriber to hold a step longer.

Does the table show doses or dates first?

Each row is one week: week number, the Monday-to-Sunday dates from your start date, and the weekly dose. Step changes are highlighted, and the first date of each new step is listed separately.

Where can I see how a ladder changes modeled levels?

Open the same ladder in the Tirzepatide plotter: it draws each step above the modeled amount so you can see the build-up after each increase.

Is tirzepatide FDA approved?

Yes. FDA approved as Mounjaro (type 2 diabetes) and Zepbound (weight management, obstructive sleep apnea). Numbers on this page quote the FDA prescribing information; your prescriber sets your dose.
