Short answer
Semaglutide's half-life is about 1 week. Each week without a dose halves what remains (0.5 raised to days ÷ 7): 50% after 7 days, 25% after 14 days, about 3% after 35 days. Labels say it persists about 5 to 7 weeks (Wegovy) or 5 weeks (Ozempic) after the last dose, as of Oct 19, 2026.

- First peak about 36 hours after the first dose (time to peak).
- Trough: the lowest point, just before the next weekly dose.
- Accumulation: each dose adds to what is left, so peaks and troughs climb toward about 2× a single dose.
- Stable window: from about day 28, the weekly trough changes by less than 5% from one week to the next.
- Washout: after the last dose the amount halves every 7 days.

Half-life is the time it takes for the amount of a drug in the body to fall by half once absorption is over. For semaglutide, both FDA labels put it at about 1 week. That one number explains the weekly schedule, the slow build-up over the first month and the long tail after the last dose.
Semaglutide pharmacokinetics at a glance
| Measure | Value | Label |
|---|---|---|
| Elimination half-life | About 1 week | Wegovy, Ozempic |
| Time to maximum concentration | 1 to 3 days | Wegovy, Ozempic |
| Absolute bioavailability, injection | 89% | Wegovy, Ozempic |
| Absolute bioavailability, tablets | About 1% to 2% | Wegovy |
| Time to steady state, weekly dosing | 4 to 5 weeks | Ozempic |
| Present after last dose | About 5 to 7 weeks | Wegovy |
| Present after last dose | About 5 weeks | Ozempic |
| Volume of distribution | About 12.5 L | Wegovy |
| Apparent clearance | About 0.05 L/h | Wegovy |
| Plasma albumin binding | More than 99% | Wegovy |
| Steady-state concentration, 2.4 mg weekly | About 75 nmol/L | Wegovy |
| Steady-state concentration, 7.2 mg weekly | About 230 nmol/L | Wegovy |
| Steady-state concentration, 0.5 mg / 1 mg | About 65 / 123 ng/mL | Ozempic |
Two notes on the table. The Wegovy label says steady-state exposure rose in proportion to dose up to 7.2 mg once weekly. And injection site does not change much: exposure was similar in the abdomen, thigh and upper arm.
How much is left after a single dose
The table below comes from the site’s model. It shows a single 1 mg dose so the numbers read as fractions. The “modeled amount” column includes absorption (peak around day 1.5); the “left by decay alone” column is pure halving every 7 days.
| Day | Modeled amount (mg-eq) | Half-lives elapsed | Left by decay alone |
|---|---|---|---|
| 1 | 0.8275 | 0.14 | 90.6% |
| 3 | 0.7772 | 0.43 | 74.3% |
| 7 | 0.5241 | 1 | 50% |
| 14 | 0.2621 | 2 | 25% |
| 21 | 0.131 | 3 | 12.5% |
| 28 | 0.0655 | 4 | 6.3% |
| 35 | 0.0328 | 5 | 3.1% |
| 49 | 0.0082 | 7 | 0.8% |
| Days since dose | Half-lives | Left | Cleared |
|---|---|---|---|
| 7 | 1 | 50% | 50% |
| 14 | 2 | 25% | 75% |
| 21 | 3 | 12.5% | 87.5% |
| 28 | 4 | 6.25% | 93.75% |
| 35 | 5 | 3.1% | 96.9% |
| 49 | 7 | 0.8% | 99.2% |
With absorption included, the model gives 0.5241 mg-equivalents left on day 7 and 0.0328 on day 35 from a 1 mg dose. Those are close to the pure-decay figures because semaglutide is absorbed within days but eliminated over weeks.
How long semaglutide stays in your system
A common rule of thumb is that a drug is mostly gone after about 5 half-lives, when roughly 97% has been cleared. For semaglutide, 5 half-lives is about 35 days, or 5 weeks. The labels say the same thing in their own words:
- Wegovy: semaglutide “will be present in the circulation for about 5 to 7 weeks after the last injectable dose of 2.4 mg or 7.2 mg” or the last 25 mg tablet. (Novo Nordisk, Jun 18, 2026)
- Ozempic: semaglutide “will be present in the circulation for about 5 weeks after the last dose.” (Novo Nordisk, Jun 1, 2026)
The labels do not explain why the two ranges differ, and the Wegovy figure is tied to its higher maintenance doses. Talk to your prescriber before stopping, pausing or restarting.
Steady state and accumulation
Because a new dose arrives before the last one is gone, the amount in the body builds up over the first weeks at a dose. Under the site’s model, the weekly accumulation ratio for semaglutide is 2.0000. In plain terms: at steady state, the amount in the body at any point in the week is about twice what a single dose would leave at that same point. It follows directly from half of each dose remaining after 7 days.
The Ozempic label states steady state is reached after 4 to 5 weeks of weekly dosing. (Novo Nordisk, Jun 1, 2026) The Wegovy ladder changes dose every 4 weeks, so during escalation the body is usually still approaching one plateau when the next step starts. The chart below shows that pattern.
- Weekly dose (published steps)
- Modeled amount (mg-eq)
- Stable window
Each 4-week step lifts the modeled curve, and the curve only levels out during the hold at 2.4 mg. Try other schedules in the semaglutide plotter, or map the label steps to dates with the titration calculator.
Build-up and wash-out week by week
The same halving drives both the climb to steady state and the fall after the last dose. At a fixed weekly dose, the share of the eventual steady-state level reached by the end of each week is 1 minus 0.5 raised to the number of weeks. After stopping, the amount falls by half each week.
| Week | Share of steady state reached | Share left after stopping |
|---|---|---|
| 1 | 50% | 50% |
| 2 | 75% | 25% |
| 3 | 87.5% | 12.5% |
| 4 | 93.75% | 6.25% |
| 5 | 96.9% | 3.1% |
| 6 | 98.4% | 1.6% |
The build-up column lands at about 94% to 97% by weeks 4 and 5, which lines up with the Ozempic label’s statement that steady state is reached after 4 to 5 weeks. (Novo Nordisk, Jun 1, 2026) The wash-out column mirrors it, which is consistent with the labels’ figure of about 5 weeks or more in circulation after the last dose.
Half-life and the label’s timing rules
Several label rules involve time windows. The labels do not explain their reasoning, but the windows are easier to remember next to a 1-week half-life.
| Rule | Window | Label |
|---|---|---|
| Missed weekly injection | Give if next dose is more than 2 days away | Wegovy |
| Missed weekly injection | Give within 5 days of the missed dose | Ozempic |
| Two or more missed injections in a row | Restart escalation at a lower dose | Wegovy |
| Changing the weekly dosing day | At least 2 days (over 48 hours) between doses | Ozempic |
| Switching injection to tablets | Start 25 mg tablets one week after last 2.4 mg injection | Wegovy |
The full ladders are in the semaglutide dosage chart.
Injection vs tablet
The tablet and the injection deliver the same molecule, but very different amounts reach the blood. Absolute bioavailability is 89% after subcutaneous injection and about 1% to 2% for the tablet, which uses an absorption enhancer (SNAC) in its formulation. (Novo Nordisk, Jun 18, 2026) That is why the tablet ladder runs from 1.5 mg to 25 mg daily, while the injection ladder runs from 0.25 mg to 2.4 mg weekly. The label’s 5-to-7-week clearance figure covers the 25 mg tablet as well.
What the model does and does not show
The site’s charts use a one-compartment model with first-order absorption (the Bateman function). It uses a 7-day half-life and a 36-hour tmax, a point inside the label’s 1-to-3-day range. Output is a relative amount in mg-equivalents with bioavailability set to 1, not a blood concentration. Real levels vary from person to person. Full details are on the model card, and approvals and trial data are on the semaglutide hub.
Frequently asked questions
What is the half-life of semaglutide?
- Both the Wegovy and Ozempic labels state an elimination half-life of approximately 1 week, and both injections are dosed once weekly. Peak concentration comes 1 to 3 days after a dose.
How long does semaglutide stay in your system?
- The Wegovy label says semaglutide is present in the circulation for about 5 to 7 weeks after the last 2.4 mg or 7.2 mg injection or 25 mg tablet. The Ozempic label says about 5 weeks after the last dose. With a 1-week half-life, about 3% of a dose remains after 5 weeks by decay alone.
How long does it take Ozempic to reach steady state?
- The Ozempic label states that steady-state exposure is reached after 4 to 5 weeks of once-weekly dosing. The Wegovy label reports steady-state concentrations (about 75 nmol/L at 2.4 mg) but does not state a time to steady state. Each dose increase starts a new approach to a higher steady state.
How long does Wegovy stay in your body after stopping?
- About 5 to 7 weeks after the last 2.4 mg or 7.2 mg injectable dose or 25 mg oral dose, per the Wegovy label. Talk to your prescriber before stopping or restarting, because the label has specific restart rules after 2 or more missed doses.
Does the semaglutide pill have the same half-life?
- The Wegovy label's 5-to-7-week figure covers the 25 mg tablet as well as the injections. What differs is absorption: absolute bioavailability is about 1% to 2% for the tablet vs 89% for the injection, which is why the tablet doses are much larger in mg.
How long does Ozempic take to wear off?
- The Ozempic label says semaglutide remains in the circulation for about 5 weeks after the last dose. By simple half-life arithmetic, about half of the amount present at the last dose is left one week later, a quarter after two weeks and about 3% after five weeks. How quickly effects fade for a given person is not stated in the label; ask your prescriber.
