Short answer

Each label sets its own rule. As of Oct 21, 2026: Ozempic allows 5 days, Zepbound and Mounjaro 4 days (96 hours), and Wegovy injection only if the next dose is more than 2 days away. Example: a Zepbound dose due Monday at 8 a.m. can be taken until Friday at 8 a.m.; after that, skip it.

Every FDA-approved GLP-1 brand has its own missed-dose rule, and they do not match. Two brands with the same drug inside, Wegovy and Ozempic, word their rules differently. This page puts the rules side by side and quotes each prescribing information as posted on DailyMed when we checked on Oct 21, 2026. It is a reference, not advice: if you have missed a dose and are unsure what to do, talk to your prescriber or pharmacist.

“No separate rule” means the label says nothing about a run of missed doses; it does not mean a long gap is safe to ignore. The weekly windows run from 4 days (tirzepatide) to 5 days (semaglutide); Wegovy states its rule from the next dose instead. Only two labels, Wegovy injection and Foundayo, require a restart at a lower dose. And the two daily pills handle a missed dose in opposite ways.

Wegovy injection and tablets

The Wegovy prescribing information (revised 6/2026) covers both the weekly injection and the daily tablet, and section 2.4 gives each its own rule (Novo Nordisk, Jun 18, 2026). For the injection, the label says that if one dose is missed and the next scheduled dose is “more than 2 days away, administer WEGOVY injection as soon as possible.” If the next dose is “less than 2 days away, do not administer the WEGOVY injection dose. Resume dosing on the regularly scheduled day of the week.”

The label then adds a restart rule that no other weekly brand has: “If 2 or more consecutive doses of WEGOVY injection are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions” (Novo Nordisk, Jun 18, 2026). It does not name the restart dose. The patient information says the same thing in plainer words: contact your healthcare provider “to talk about how to restart your treatment.”

Wegovy injection missed-dose examples for a Monday dosing day (our arithmetic on label section 2.4) (Novo Nordisk, Jun 18, 2026)
You remember onDays until next Monday doseWhat the label says
Tuesday6Take it as soon as possible
Thursday4Take it as soon as possible
Saturday nightLess than 2Skip it; resume Monday
After 2 Mondays missed in a rowNot applicableRestart escalation at a lower dose, set by your prescriber

For the tablets, the rule is a skip: “If a dose of WEGOVY tablets is missed, skip the missed dose and take the next dose the following day” (Novo Nordisk, Jun 18, 2026). Section 2 also says “Do not take more than one tablet per day.” The 2-day rule for the injection is not new; the original 2021 Wegovy label used the same window (FDA, Jun 4, 2021). The full ladder is on the Wegovy dosing schedule page.

Ozempic

Ozempic, the type 2 diabetes brand of semaglutide, states its window as a plain count of days. Section 2.1 says: “If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once-weekly dosing schedule” (Novo Nordisk, Jun 1, 2026).

Example: your dose day is Sunday. On Wednesday, 3 days late, the label says to take it, then go back to Sunday. On the following Saturday, 6 days late, you skip it and take the next dose on Sunday. The Ozempic label has no restart rule for several missed weeks. Wegovy words its rule from the next dose (more than 2 days away); on a weekly schedule that works out to the same 5-day cutoff, though only Wegovy adds a restart rule. More detail is on the Ozempic dosing schedule page.

Zepbound and Mounjaro

The two tirzepatide brands share the same wording. Zepbound section 2.3 says: “If a dose is missed, instruct patients to administer ZEPBOUND as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once weekly dosing schedule” (DailyMed, Aug 28, 2026). The Mounjaro label repeats this word for word, with the brand name changed (DailyMed, Aug 27, 2026).

The patient information in both labels adds a spacing limit: “Do not take 2 doses of ZEPBOUND within 3 days (72 hours) of each other” (DailyMed, Aug 28, 2026). The Mounjaro version reads “within 3 days of each other” (DailyMed, Aug 27, 2026).

A worked example: if the usual dose is Monday at 8 a.m., the 96-hour window closes Friday at 8 a.m. A dose taken Friday at 8 a.m. sits exactly 3 days (72 hours) before the next Monday dose. Neither tirzepatide label has a rule for 2 or more missed weeks, and neither says to restart lower. See the Zepbound dosing schedule and Mounjaro dosing schedule for the full ladders.

Foundayo

Foundayo (orforglipron) is a once-daily GLP-1 tablet from Eli Lilly. Section 2.3 of its label (version effective Jul 29, 2026) says: “If a dose is missed, instruct patients to take the dose as soon as possible. Advise patients not to double up the next dose. If 7 or more consecutive doses are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions” (Eli Lilly, Jul 29, 2026).

The patient information adds: “Do not take 2 doses of FOUNDAYO in the same day,” and if you miss it “for 7 or more days in a row, call your healthcare provider to talk about how to restart your treatment” (Eli Lilly, Jul 29, 2026). The Foundayo dosing schedule covers its 30-day steps.

Why the rules differ

The labels state their rules but not the reasoning behind each window. Two things in the labels help explain the pattern: how often the drug is taken, and how long it stays in the body.

Dosing interval and half-life by drug, from each label (Novo Nordisk, Jun 18, 2026) (Novo Nordisk, Jun 1, 2026) (DailyMed, Aug 28, 2026) (DailyMed, Aug 27, 2026) (Eli Lilly, Jul 29, 2026)
Drug (brands)IntervalElimination half-life in the labelWeekly missed-dose window
Semaglutide (Wegovy, Ozempic)Weekly injection or daily tabletApproximately 1 week5 days (Ozempic); next dose more than 2 days away (Wegovy)
Tirzepatide (Zepbound, Mounjaro)Weekly injectionApproximately 5 days (Mounjaro); 5 to 6 days (Zepbound)4 days (96 hours)
Orforglipron (Foundayo)Daily tabletApproximately 29 to 49 hoursNot applicable

Weekly drugs with long half-lives. A half-life is the time for the amount in the body to fall by half. With a half-life near a week, much of the last dose is still present a few days late. Our one-compartment model puts the share of a weekly dose left after 7 days at 0.50 for semaglutide (7-day half-life) and about 0.38 for tirzepatide (5-day half-life). That is a modeled relative amount, not a blood level. You can see how a late or skipped dose changes the curve in the semaglutide plotter or the tirzepatide plotter.

Windows that protect spacing. One pattern is simple arithmetic on the labels, not a label statement. A dose taken on the last day of the Ozempic window leaves 2 days before the next one, which matches Ozempic’s day-change minimum of at least 2 days. A dose taken at the end of the 96-hour tirzepatide window leaves 3 days (72 hours), the tirzepatide day-change minimum. Wegovy writes the rule from the other end: take it only if the next dose is more than 2 days away. In each case the label keeps two weekly doses from landing close together.

Daily pills. A daily tablet is one of 7 doses in a week, and Foundayo’s 29- to 49-hour half-life means levels respond to a gap within days. Our reading: that is why these labels deal in skipped days rather than a window. Wegovy tablets skip the missed day; Foundayo takes it late but forbids two in one day.

Restarts. Both labels with a restart rule give the same reason: “to reduce the risk of gastrointestinal adverse reactions” (Novo Nordisk, Jun 18, 2026) (Eli Lilly, Jul 29, 2026). The thresholds differ, 2 weekly doses for the Wegovy injection versus 7 daily doses for Foundayo. To see where a lower restart step would sit on a calendar, try the semaglutide titration tool or the tirzepatide titration tool, with a restart plan from your prescriber.

Changing your dosing day

Four weekly labels say when the injection day may move (Wegovy only in its patient information).

Rules for changing the weekly dosing day, from each label (Novo Nordisk, Jun 1, 2026) (DailyMed, Aug 28, 2026) (DailyMed, Aug 27, 2026) (Novo Nordisk, Jun 18, 2026)
BrandLabel wordingMinimum gap
Ozempic“as long as the time between two doses is at least 2 days (>48 hours)”More than 48 hours
Zepbound“as long as the time between the two doses is at least 3 days (72 hours)”72 hours
MounjaroSame wording as Zepbound72 hours
Wegovy injectionPatient information only: “as long as your last dose of WEGOVY was given 2 or more days before”2 days
Wegovy tablets, FoundayoDaily; no day-change ruleNot applicable

Example under the Zepbound rule: moving from Monday to Thursday puts 3 days between doses, which the label allows; taking the next dose the very next day (Tuesday, 1 day later) does not. Tell your prescriber if you change your routine.

What the labels do not say

  • No label tells you to double a dose. Foundayo says “not to double up the next dose.” Wegovy tablets allow no more than one tablet per day, Foundayo no more than one dose per day (“Do not take 2 doses of FOUNDAYO in the same day”), and Zepbound and Mounjaro say not to take 2 doses within 3 days of each other (Eli Lilly, Jul 29, 2026) (Novo Nordisk, Jun 18, 2026) (DailyMed, Aug 28, 2026) (DailyMed, Aug 27, 2026).
  • No restart dose is named. Wegovy and Foundayo say “a lower dosage” without saying which one.
  • Ozempic, Zepbound and Mounjaro say nothing about several missed weeks. They neither require nor rule out a restart. That gap is a question for your prescriber, not a sign that any dose is fine.
  • No weekly label offers a catch-up schedule. Each one says to take the missed dose or skip it, then return to the regular day; none describes taking two weekly doses close together to make up time.
  • No label covers compounded products. Compounded semaglutide and tirzepatide are not FDA approved, and FDA does not review them for safety, effectiveness or quality before marketing (FDA, Oct 1, 2026). Brand label rules do not transfer to them.

Retatrutide has no missed-dose rule

Retatrutide is investigational. Lilly said on Aug 12, 2026 that no medicine containing retatrutide had been approved for human use by any regulator (Eli Lilly, Aug 12, 2026). With no FDA label, there is no approved missed-dose rule to quote, and trial protocols are not instructions for anyone outside a trial. See is retatrutide FDA approved for the dated status.

For the label facts behind each drug, see the semaglutide hub and the tirzepatide hub. Any decision about a missed, late or restarted dose belongs with your prescriber.

Frequently asked questions

What happens if I miss a dose of Mounjaro?

The Mounjaro label says to take the missed dose as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip it and take the next dose on the regularly scheduled day. The patient information adds: do not take 2 doses of Mounjaro within 3 days of each other. The label has no separate rule for several missed weeks, so talk to your prescriber about a longer gap.

What happens if I miss my Ozempic dose?

Per the Ozempic label, take it as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip it and take the next dose on the regularly scheduled day, then continue once weekly. Wegovy, the other weekly semaglutide brand, words its rule differently: it counts from the next dose (more than 2 days away) and adds a restart rule. Ask your pharmacist or prescriber if you are unsure.

Can I take Wegovy a day late?

For the weekly injection, the Wegovy label ties the decision to the next dose. If the next scheduled dose is more than 2 days away, take the missed dose as soon as possible; a dose one day late leaves about 6 days, so it falls in that case. If the next dose is less than 2 days away, skip it. For Wegovy tablets, a missed daily dose is skipped and the next one taken the following day.

What if I miss 2 weeks of Zepbound?

The Zepbound label covers a single missed dose: take it within 4 days (96 hours), otherwise skip it and resume on the regular day. It gives no separate instruction for 2 or more missed weeks and does not say whether to restart at a lower dose. That differs from Wegovy, whose label says to restart escalation after 2 or more consecutive missed doses. Call your prescriber before your next dose after a 2-week gap.

Can I take a missed GLP-1 dose late?

Sometimes, within the window each label sets. Ozempic allows 5 days, Zepbound and Mounjaro 4 days (96 hours), and Wegovy injection only if the next dose is more than 2 days away. Foundayo, a daily pill, says take it as soon as possible but never double up; a missed Wegovy tablet is skipped instead. Outside those windows, the labels say to skip the dose. Talk to your prescriber about anything else.
