Ozempic is semaglutide approved for type 2 diabetes. Novo Nordisk sells it through two price routes: a copay savings card for people whose commercial plan covers Ozempic, and NovoCare Pharmacy self-pay for everyone else. All figures below are as shown on Novo Nordisk pages on Oct 19, 2026, and rechecked on Oct 20, 2026; programs change.

Who qualifies for the Ozempic savings card

The ozempic.com savings page sets the main conditions (Novo Nordisk, Oct 19, 2026):

  • Commercial insurance that covers Ozempic. The $25 offer is for commercially insured patients. If your plan does not cover Ozempic, the card does not lower a cash price.
  • No government drug coverage. The page says government beneficiaries are excluded. It names Medicare beneficiaries with Part D coverage and people enrolled in federal or state programs such as Medicaid.
  • Any pen dose. The offer applies to any dose of the Ozempic pen, for up to a 3-month prescription.
  • Time limit. It “provides up to $100 in monthly savings and is valid for up to 48 months.”
  • Changes allowed. Novo Nordisk reserves the right to modify or cancel its programs at any time.

How the $25 floor and $100 cap work

The card reduces your copay by up to $100 a month and will not take it below $25. Plans with high copays or a deductible still leave a balance.

Illustrative monthly out-of-pocket cost with the Ozempic savings card (our arithmetic on the published $25 floor and $100 cap)
Your plan copayCard paysYou pay
$40$15$25
$100$75$25
$125$100$25
$140$100$40
$250$100$150

These rows are worked examples, not quotes from Novo. Your plan sets the copay; the card only applies the cap and floor. Over 48 months at the $25 floor, the out-of-pocket total would be 48 × $25 = $1,200, if your copay stayed at or under $125 and the program did not change.

Self-pay prices through NovoCare Pharmacy

If your plan does not cover Ozempic, or you have Medicare or Medicaid, the self-pay route applies (Novo Nordisk, Oct 19, 2026):

  • New patients pay $199 per month for each of the first two monthly fills of 0.25 mg and 0.5 mg. The offer runs through Dec 31, 2026.
  • After that, the price is $349 per month for 0.25, 0.5 or 1 mg and $499 per month for 2 mg.
  • One month is one pen box.

The $199 introductory price and $349 standard price trace to November 2025 (Novo Nordisk, Feb 2026). The ozempic.com page notes that self-pay purchases happen outside your insurance plan and cannot be submitted for reimbursement or counted toward your deductible or out-of-pocket limit (Novo Nordisk, Oct 19, 2026).

Which pen each price buys

The Ozempic label lists three 3 mL pens, each tied to specific doses (Novo Nordisk, Jun 1, 2026). The self-pay price follows the dose, so the pen you are prescribed decides the tier.

Ozempic pens, the doses each delivers, and the self-pay tier as shown on Oct 19, 2026 (Novo Nordisk, Jun 1, 2026) (Novo Nordisk, Oct 19, 2026)
Pen (strength per pen)Doses per penSelf-pay tier
2 mg/3 mL (0.68 mg/mL)4 doses of 0.25 mg and 2 of 0.5 mg, or 4 doses of 0.5 mg$199 intro, then $349
4 mg/3 mL (1.34 mg/mL)4 doses of 1 mg$349
8 mg/3 mL (2.68 mg/mL)4 doses of 2 mg$499

A 12-month cost example

These totals are our arithmetic on the Oct 19, 2026 self-pay prices. They assume a new patient who follows the label’s minimum steps: 0.25 mg for 4 weeks, then 0.5 mg, with each later increase only if more glycemic control is needed after at least 4 weeks (Novo Nordisk, Jun 1, 2026). A month is treated as one pen box.

0.5 mg2 × $19910 × $349$3,888

2 mg (from month 4)2 × $1991 × $349 + 9 × $499$5,238
Savings card at $25 floor12 × $25included$300

The label reserves 2 mg for when additional glycemic control is needed after at least 4 weeks on 1 mg. See the Ozempic dosing schedule for the full ladder, and time the steps with the semaglutide titration calculator.

List price now and in 2027

On Feb 24, 2026, Novo Nordisk announced a list price of $675 per month for Ozempic, Wegovy and Rybelsus, effective Jan 1, 2027 (Novo Nordisk, Feb 24, 2026). The same release says the change does “not have an impact on direct-to-patient, self-pay prices.”

We did not find the current Ozempic list price in dollars on the Novo pages we checked, so we do not state it. A lower list price mainly matters if you pay coinsurance or are in your deductible.

Medicare, Medicaid and Ozempic

  • Savings card: not available to Medicare Part D or Medicaid enrollees (Novo Nordisk, Oct 19, 2026).
  • Self-pay: available to these patients, outside their plan.
  • Spreading costs: the ozempic.com page points Medicare patients to the Medicare Prescription Payment Plan (M3P), as an option for spreading out-of-pocket drug costs.
  • Patient assistance: the same page says the NovoCare Patient Assistance Program may provide free Ozempic to eligible patients, with income and U.S. residency requirements.
  • The $50 Medicare GLP-1 Bridge described by Novo covers Wegovy injection and pill (Novo Nordisk, Jul 1, 2026). The pages we reviewed do not list Ozempic in it.

Ask your Part D plan how Ozempic is covered for your diagnosis.

Ozempic pill prices

Ozempic.com also lists an Ozempic pill with its own price routes. One month is defined as 1 bottle of 30 tablets (Novo Nordisk, Oct 19, 2026).

Government beneficiaries are excluded from the $25 offer, as with the pen. This guide focuses on the injectable pen; ask your prescriber whether the tablet applies to you.

The two brands are approved for different conditions and are not interchangeable without a new prescription. Compounded semaglutide is a third, separate category: FDA says compounded drugs are not FDA approved (FDA, Apr 1, 2026), and none of these programs apply to them.

Before you enroll

  1. Ask your plan whether Ozempic is covered and whether prior authorization applies.
  2. If covered, activate the savings card through Novo Nordisk and give the details to your pharmacy.
  3. If not covered, compare the self-pay price for your dose with your plan price.
  4. Talk to your prescriber before changing dose or brand to fit a price tier.

For the Wegovy version of these terms, see the Wegovy savings card guide. For tirzepatide, see the Mounjaro savings card guide.

Frequently asked questions

How much is Ozempic with the savings card?

As shown on ozempic.com on Oct 19, 2026, people with commercial insurance that covers Ozempic can pay as little as $25 for any pen dose, for up to a 3-month prescription. The card provides up to $100 in monthly savings and is valid for up to 48 months. If your copay is $140, for example, the card pays $100 and you pay $40. Programs change, so check the current terms.

Does Medicare cover Ozempic with the savings card?

No. Ozempic.com states that Medicare beneficiaries with Part D coverage are not eligible for the pay-as-little-as-$25 offer, and that people enrolled in federal or state programs such as Medicaid are also excluded. Those patients can use Novo Nordisk's self-pay prices instead, but self-pay purchases sit outside the plan and do not count toward a deductible. Ask your Part D plan about your Ozempic coverage.

How much is Ozempic without insurance?

Through NovoCare Pharmacy, as shown on Oct 19, 2026: new patients pay $199 a month for the first two monthly fills of 0.25 or 0.5 mg (offer through Dec 31, 2026). How long does the Ozempic savings card last?

Ozempic.com says the commercial savings offer is valid for up to 48 months, with up to $100 in monthly savings. Novo Nordisk also reserves the right to modify or cancel its programs at any time, so the 48 months is a maximum, not a guarantee. The separate $199 new-patient self-pay price ends for prescriptions after Dec 31, 2026.

Is Ozempic cheaper than Wegovy?

Both have the same $25 floor and $100 monthly cap on the commercial savings card, and the same $199 intro and $349 standard self-pay price for most doses, as shown on Oct 19, 2026. They differ at the top dose: the Ozempic 2 mg pen is $499 a month self-pay, while Wegovy 2.4 mg is $349 and Wegovy HD 7.2 mg is $399. They are approved for different uses, so ask your prescriber which applies.
